A group of college students, who were on their way to their own graduation, get injured in a car crash and are admitted to the ER while Meredith, Derek and the rest of the surgical wing are preparing for the Der-Mer wedding. And Izzie fears that her cancer may be back, as she experiences familiar symptoms.
This episode's title refers to the song, What a Diff'rence a Day Made, by American jazz singer Dinah Washington.
